FRAILTY VERSUS ASA CLASSIFICATION: WHICH BETTER PREDICTS POSTOPERATIVE ICU ADMISSION? : A SYSTEMATIC REVIEW
Keywords:
Frailty, ASA classification, postoperative ICU admission, preoperative risk assessment, systematic reviewAbstract
Background: Preoperative risk stratification is crucial for optimizing postoperative outcomes, particularly for predicting intensive care unit (ICU) admission. The American Society of Anesthesiologists (ASA) Physical Status Classification System is widely used but subjective and not designed to assess physiologic reserve. Frailty, a multidimensional syndrome of decreased reserve, is a promising alternative. This systematic review compares the predictive performance of frailty indices versus the ASA classification for postoperative ICU admission in adult surgical patients.
Methods: A systematic review was conducted following PRISMA guidelines. Studies were screened for dual assessment of frailty and ASA, adult surgical populations (≥18 years), reporting of ICU admission or related severe outcomes, and comparative statistical data. Data extraction focused on predictive performance metrics (odds ratios [OR], area under the curve [AUC]), study characteristics, and direct comparisons between frailty and ASA.
Results: Frailty indices consistently demonstrated significant positive associations with adverse outcomes. In orthopaedic trauma (n=36,424), the modified Frailty Index (MFI) showed a stronger association with 30-day mortality (aOR 2.6) than ASA (aOR 2.5) [1]. In hip fracture patients, the Clinical Frailty Scale (CFS) had superior discriminative ability for mortality (AUC 0.699) compared to ASA (AUC 0.634) [2]. Frailty was the dominant predictor of mortality and serious complications in head and neck surgery [3] and vascular surgery (OR 2.058, p<0.001) [30]. For ICU-specific outcomes, frailty was associated with significantly higher odds of ICU admission in gynecologic oncology (OR 3.99) [6] and longer ICU stays in cardiac surgery [50]. Only one study (pituitary surgery) found ASA superior for rare ICU-level complications [5], while a hip fracture study found all scores performed poorly [7].
Discussion: The superiority of frailty over ASA stems from frailty’s multidimensional nature, capturing sarcopenia, cognitive impairment, and functional decline that determine recovery capacity. ASA, a subjective global rating, conflates stable comorbidities with acute issues and lacks reserve assessment. Context matters: frailty outperforms ASA in low-acuity and elective procedures but both are important in high-acuity emergencies. Combined models (e.g., Milan Bio-Surgical Score) show excellent predictive power (AUC 0.965) [17].
Conclusion: Frailty indices significantly outperform ASA classification for predicting postoperative ICU admission and related adverse outcomes across most surgical populations. Frailty assessment should be implemented as a standard adjunct to ASA for preoperative risk stratification and ICU planning.
