Abstract

Introduction: Post-traumatic stress disorder (PTSD) occurs in preschool children following abuse, accidents, or medical procedures. Trauma-focused cognitive behavioral therapy (TF-CBT) is evidence-based for school-aged children, but developmental adaptations for preschoolers are needed. This review evaluates TF-CBT outcomes in children aged 3–6 years.

Methods: We searched MEDLINE, PsycINFO, and PILOTS database for studies of TF-CBT (or manualized adaptations: TF-CBT-P, PCIT-T) in preschoolers with PTSD symptoms. Primary outcome: PTSD symptom reduction (clinician-administered). Secondary: behavioral problems, caregiver distress. Randomized and quasi-experimental designs included. Meta-analysis using random-effects.

Results: Seven studies (n=418 preschoolers) met inclusion: 4 RCTs, 3 open trials. TF-CBT significantly reduced PTSD symptoms compared to waitlist or treatment as usual (SMD –1.12, 95% CI –1.48 to –0.76, p<0.001). Remission (loss of diagnosis) occurred in 68% of treated children vs. 22% controls. Behavioral problems (CBCL) improved moderately (SMD –0.54). Caregiver distress improved (SMD –0.49). Adaptations included: parental involvement in all sessions, play-based exposure (drawing, puppets), shorter sessions (30 min). No study reported worsening of symptoms or harm.

Discussion: TF-CBT is highly effective for preschool PTSD when developmentally adapted. The large effect size exceeds that reported in older children, possibly due to greater parental involvement and malleability of young children's fear networks. Limitations: small number of studies, predominantly white samples.

Conclusions: Trauma-focused CBT adapted for preschoolers effectively treats PTSD and comorbid behavioral problems. Widespread implementation requires training clinicians in play-based techniques and co-therapy with caregivers.
