Introduction: Women with gestational diabetes (GDM) are at high risk for postpartum weight retention and subsequent type 2 diabetes. Lifestyle intervention efficacy in this specific population is unclear.
Methods: Systematic review of randomized controlled trials of diet/exercise interventions starting within 6 months postpartum in women with recent GDM. Primary outcome: weight change from baseline to 12 months. Secondary: diabetes incidence.
Results: Eight trials (n=2,100 women) were included. Intervention groups lost a mean of 1.8 kg more than controls at 12 months. Breastfeeding duration was longer in intervention arms (mean difference 2.1 months). Diabetes incidence at 2 years was reduced from 11% to 7% (risk ratio 0.64). Dropout rates were high (32%).
Discussion: Modest weight loss is achievable, but many interventions are intensive. The diabetes prevention effect is promising but based on few events. Sustaining lifestyle changes beyond one year remains a challenge.
Conclusions: Postpartum lifestyle interventions in GDM women produce modest weight loss and may reduce diabetes risk. Scalable, less intensive interventions are needed for widespread implementation.
